Internet
Registration Form.

2. Print
and sign the form indicating you will comply with our Rules and
Regulations and provide all requested information.
3. Download
and complete the
Student and Dog Profile Form.

Information provided helps place your dog in the appropriate
class group.
4. Mail
the following to DOTC Registrar at address below:
- Your check for $85 payable to "DOTC"
- Completed and signed Internet Registration Form
(Current Rabies Vaccination Information Required for Class Entry)
- Completed Student and Dog Profile Form
